16 Shaw - was replaced for Bradford City v Lincoln 1984-85 late on by Norman Glover (28)
and was ref replaced after 80 mins at Preston v Blackpool 76-77 which led to Graham Kelly stepping in as a line

17 David Hutchinson - Liverpool v Arsenal 1988-89 title decider

18 David S Hutchinson - Highgate United v Enfield Amateur Cup 66-67 Tony Allden killed by lightning

Yes, I was thinking of Man City v Luton for Arnold Challinor. Luton scored with 5 minutes to go to send Man City down and keep Luton up, and the Luton manager rushed on to the pitch at the end, resplendent in a cream suit. It's on YouTube of course.
