The earliest example of this I've found for Leeds matches is was 23rd November 1957 Burnley v Leeds, Mr C.H. Sant didn't emerge for second half another referee who just happened to be at the match took over Mr R.H. Speake.
3rd September 1960 Southampton v Leeds A.J. Sturgeon was injured in the game don't know which linesman or ref took his place
25th November 1961 Leeds v Walsall ref R.H. Windle had to retire so F.W. Cooper took his place
28th November 1970 Leeds v Manchester City A. Dimond was replaced by linesman D.N. Harwood
Then the most bizarre example 22nd December 1973 Leeds v Norwich City, referee Dennis Turner was hurt during the game, one of his linemen Mr T Carter took over the ref duties and a call went out for a linesman someone with FA accreditation badges, and the call was answered by a Leeds United car park attendant, I don't know his name but it has to be a first.
