Fred Kirkham is in twice
Another understandable one with two F. Kirkham's with a slight gap of dates and different locations

He had 10 seasons based at Preston (1897-98 to 1906-07) and refereeing the 1906 F.A. Cup final and 11 internationals

On 13th Apr 1907 he refereed Tottenham v Watford in the Southern League, nine days later he was unveiled as the new Tottenham manager

He apparently was not popular with players or fans but was part of the delegation to the F.L. in late June to take Stoke's place in league

Stoke had resigned, re-formed & re-applied to the F.L. so should Stoke count as being formed in 1863 (or 1868 depending upon the source)?

Anyway Tottenham got Stoke's F.L. place but Kirkham resigned three weeks later before Tottenham played a F.L. match

He was also too late to rejoin the ref's list for 1908-09 but he appears to have taken some matches in Ireland

He re-joined in 1909-10 (as a supplementary voted in by the management committee) when he was still based at Enfield after his Tottenham season

By 1910-11 (still a supplementary) he had moved back north to the outskirts of Ilkley/Bradford at Ben Rhydding (which sounds more like a Scottish mountain) and he was still there in 1911-12 when was back on the full list for the last time although he refereed his 12th international in Antwerp in 1913 when Belgium met the Netherlands

He returned to Lancashire and took on the chairmanship of the Northern Professional Golfers Association in 1913 which he held for 23 years until his death the day before his 71st birthday
