But what was the 4O doing, they are meant to warn her that she has had done this.

Does the Senior AR still keep records of cautions and dismissals?

If so - that's 3 officials who have missed that - not good at all.

We always did in my day in case we had to take over the game. I don't know what happens now. I would assume that's 4O's duties, if a 4O is appointed.

Very strange.  Lucy Oliver gave the yellow and the player looked as though she was walking off regardless.  Oliver checked her records and must have decided that she had cautioned someone else previously.

What level is Lucy at bmb?  I assume this was quite a big game.

The strange thing is that after the second yellow card the referee appeared to reach in her pocket for a red but then apparently changed her mind. She will have to do her time on the naughty step now I guess

Was Level 3 but no longer officiating on men's game.  Women's path have a new pyramid numbering.

In every game here in NSW each official must keep a full record of the game. Not good enough at that level.

That is certainly a very fair interpretation of what appeared to happen, but only the referee and the player will know for certain.

What is baffling is that they have comms at that level. The 4th officials will always record cautions, but usually one of the assistants will be asked to as well.  Unless the comms wasn't working I just can't understand how one of the other officials hasn't spotted this and stopped her making such a monumental mistake.  It might have been more understandable had either of the cards been "flashed" at a group of players, but on both it was very clear who had been cautioned.
